this could have been fixed already with your additions to the case sensitive issues but I've requested "DAvantage - Dying Sphere (mutation engine) a number of times and had it fail to play.

Requested Epsilon Minus - Just Another Long Shot (Glis Remix). It was listed as being requested by me for about 10 mins then it vanished and the "you have already made a request today" message was displayed, even tho it didn't play.
